Who is the new WellSpan Occupational Health lab partner?

WellSpan Occupational Health will partner with Clinical Reference Lab (CRL) and FormFox, effective August 21, 2021. CRL is a testing laboratory focused on accurate testing, rapid turn-around times, and innovative systems and solutions. They are a SAMHSA certified laboratory based in Lenexa, Kansas. FormFox is a leading electronic custody and control solution in the Occupational Health industry based in Salt Lake City, Utah.

How do I know if this change impacts me?

This change only impacts employers who use WellSpan Occupational Health as their Medical Review Officer (MRO).

Why are we making this change?

CRL and FormFox will bring many benefits to you as our client including better turn-around times, online ordering and pre-registration, enhanced communications, greater technological capabilities and streamlined use with additional services in the future.

Where will this transition occur?

All seven WellSpan Occupational Health locations will now offer the same drug screen platform and services.

What does this new vendor offer?

The Interactive FormFox WorkForce Web Portal offers the following useful services:

  • Company administrator (DER) access
  • Additional company contact access based on user roles
  • Customizable notification method
  • Find testing sites
  • Order testing services
  • Track the status of services
  • View results

How do I set up a new FormFox account?

If you are a current WellSpan Occupational Health customer using us as your Medical Review Officer, an invitation to FormFox WorkForce will be emailed to the current DER on file with Occupational Health. This person will need to validate identity, enter a validation code, set a password, and set notification preferences. We recommend you say “yes” to all services. Preferences can be updated any time through the portal.

Password Requirements:

  • Minimum 8 characters in length
  • Contains ¾ of following items:
    • Uppercase Letters
    • Lowercase Letters
    • Numbers
    • Symbols

If you do not receive an email from FormFox to set up an account by August 23, 2021, please email us at occupationalhealth@wellspan.org or call 717-812-2993.

IMPORTANT: To finalize set up, a two-factor authentication process is also required.

Please send a phone number to be used for your two-factor authentication to occupationalhealth@wellspan.org. This phone number will only be used for the initial authentication process and does not need to match the number we have on file. This must be a direct phone number, without an extension or recording. We recommend a cell phone number as either text or voice can be used. Please include in your email your name and the name of your company.

What changes can I expect?

  • Lab-based panels (urine and oral)
    • No Change
  • Must contain 3 out of the 4 following items:
    • Move to closest comparable panel
    • Choices more streamlined to cover common issues in industry
    • Changes
      • Removal of propoxyphene (Darvocet) = Pain reliever. Production stopped in 2010 due to serious/lift threatening heart arrhythmia.
      • Removal of methaqualone = Sedative/hypnotic. Commercial production halted in 1980 due to widespread abuse.

Who reviews positive drug tests?

All positive drug tests will continue to be reviewed by one of WellSpan’s Medical Review Officers.

How do I order testing services in the FormFox WorkForce portal?

A tutorial is available.
Ordering tests expedites in-clinic time and drives accuracy in service. You have the option to order tests at WellSpan Occupational Health locations OR clinics outside our service area. The portal allows you to:

  • Locate clinics and collection sites which offer the service needed
  • Set timeframes the donor can have the test performed
  • Send authorization form to the donor via test or email

Is pricing the same if I schedule with a clinic outside your service area?

If scheduling testing outside of our service area, providers are categorized into tiers (platinum, gold, or silver). Pricing is determined based on the category selected.

How do I track orders in the FormFox WorkForce portal?

You can visually track the orders in a customizable status grid.

Can I still access my eScreen account?

Access to your eScreen account will end at the close of business on August 20, 2021. If you need a copy of a specimen certificate, eScreen will continue to service this need. You may reach eScreen at 800-881-0722, Option 5.

How was this change communicated?

Several communications have been sent via e-mail to employers in the region. Sometimes the e-mails do not go through due to spam filters, as the e-mails are sent via Constant Contact. We also are sending billing stuffers, adding information to our website and adding an auto-reply to our e-mail. We apologize if you are just now finding out and are doing best to transition each employer as smoothly as possible.
